A new program launching next week at the critical access hospital in Rawlins will allow Medicare patients with co-morbidities to better monitor and manage their chronic conditions. The goal is minimize hospitalizations and improve quality of life for people in the program.

Bigfoot 99’s Cali O’Hare spoke with officials at MHCC about the new program and filed this report.
